Accidental self-injection and needlestick injury appear to happen quite a lot among veterinary professionals and farm workers: cases are probably under-reported. Severe harm can result from injury involving vaccinations that contain mineral oil (they're mainly vaccines for food-production animals but there are a few for companion animals). It's really important that anyone involved in vaccinating animals is aware of which products contain mineral oil and, crucially, what to do in the case of self-injection or needlestick injury.
